Our initial consultation is free.

Upon agreement, Rest Easy Eldercare, will meet with the elderly client, and a comprehensive assessment of the individual will be performed.  This assessment will include the physical, medical, functional, social and emotional, financial and cognitive needs of the client.  Observation of current services in place and assesssment of care needs will be reviewed. 

Secondly, after a full assessment, an individualized plan of care will be developed.  This written plan of care will outline recommendations for a safe solution to the client's needs.  We will be considerate of the client's medical and psychological needs as well as the client's wishes and finances.  We will supply the caregiver with a list of specific care needs along with the appropriate services and facilities to provide them.  We can supply an in depth list of referrals appropriate for your needs at that time.

The next step is up to you. Once  Rest Easy Eldercare present you with the plan of care and referrals, you can decide to  implement  them yourself or, if agreed upon, we can research,make recommendations and arrange all services for you. 
In addition to putting these services in place for you, we will provide on-going monitoring of the services and adjust the care plan if the need arises.  We will watch over your loved one and keep you informed on a regular basis.

In todays health care environment, navigating through it can be a daunting process.  Rest Easy Eldercare nurses will act as your advocate and liaison between you and the health care professional.   We ask the right questions to get you the right answers.  We can help you understand and cope with the medical, insurance and emotional issues confronting you and your loved one.

Services we will help with:  Advocate for appropriate services or treatment, educate you and your family to managing your healthcare, review and interpret medical tests, and records for you, obtain appropriate medical services, arrange services, and arrange transportation to and from medical appointments, obtain home care nursing services, or recommend and arrange transfers to the appropriate facilities. Other services may be provided as needed after discussion with family and client.
